SIGN-UP FORM
We realize the registration form isn't the ideal way to request information regarding attending the Early Release care and Day Camp opportunities. Schedules and plans frequently change. Therefore we have created a short form we wish to use that can be completed as we approach the various dates. The form can be completed on your phone with just a few clicks. If your plans change, simple send us a new form with the correct information and we'll be notified.

A flyer that provides the URL and a scannable QR code will be on display at the Check-In kiosk
Q: Do I have to complete for all future dates?
A: No, just the upcoming dates. If you do submit for all future dates and your plans change, just submit a new form. No problem.
Q: Why do I have to submit an Early Release and Day Camp form separately?
A: Submitting separately helps us sort the information easier and track changes.
Q: Is this the only way I can communicate with KiC?
A: No, we welcome open, easy communication. However, using this method helps us better organize this information. As you can imagine, we receive a large volume of email/text/calls/message traffic everyday spanning a wide variety of topics. We are attempting to provide a process that is better for everyone.
TAX STATEMENTS
A comprehensive payment report will be sent out next month that you can use for tax and employer benefit purposes.
Can't wait? You can access your reports whenever you need them by logging into MyProcare.com, clicking REPORTS, selecting either the CUSTOMER STATEMENT (detailed report) or the CUSTOMER CHARGE CREDIT SUMMARY (higher level summary), selecting the timeframe for the report, and clicking RUN REPORT.
Our Tax ID is located in the top right corner of our reports if needed.
